(3)In addition to the above certificate which must be recorded by the members in their Travelling Allowance Bills, they should also furnish, in the following declaration Form, particulars of the dates and time of their arrival and departure, mode of conveyance used for the journey and other meetings, if any, attended by them at the place of meeting on the same day/days or within a period of three days prior to and after the meeting:Declaration
