Annual licenses to public vaccinators in areas where the licensed system of vaccination is in force shall be issued in the form (Schedule XXXI-No. 8H) which shall be signed by the Superintendent of Vaccination of the area concerned and countersigned by the Chairman of the district board concerned.A list of villages comprising the circle of the public vaccinator to whom the license has been issued shall be given in the form. The first counterfoil shall be kept in the office of the Superintendent of Vaccination and the second counterfoil shall be sent to the Assistant Director of Public Health of the area concerned.
