Background
1. The Applicant filed an RTI application dt.8.5.08 with the CPIO, DoP, Serampore. She requested for the following information with regard to SB A/c Nos. 1552152 (Sheela Pal), 775294 (Sheela Pal & Sambhu Charan Pal), 1276848 (Sheela Pal & Sikha Pal).
i) Photocopy of application forms of account holders
ii) To confirm the identity of account holders whether attested photocopy of any one of voter identity card, PAN card, Passport, driving license etc. was submitted. If so, Xerox copy to be provided
iii) To confirm address of the A/c holders whether attested Xerox copy of any one of electricity bill, phone bill, ration card, municipal tax receipt, rent receipt etc. was submitted. If so Xerox copy to be provided.
iv) Name, address, A/c Nos. of the introducers
v) At the time of opening of the accounts of the introducers whether point Nos. ii & iii were followed
vi) On the opening date of the accounts whether the account holders were present in person
vii) Who received the passbook?
viii) Xerox copy of the statement of accounts and withdrawal slips.
The CPIO replied on 19.6.08 providing information w.r.t A/c No.1276848 and with respect to other points he stated that information is being compiled and would be supplied soon. Not satisfied with the reply, the Applicant filed an appeal dt.10.9.08 with the Appellate Authority requesting for remaining information with regard to A/c No.1276848 and complete information with regard to other two accounts. On not receiving any reply from the Appellate Authority, she filed a second appeal dt.20.1.09 before CIC requesting for the information.
2. The Bench of Mrs. Annapurna Dixit, Information Commissioner, scheduled the hearing for April 13, 2009.
3. Mr. Mihir Nag, Sr. Supdt. of Post & CPIO and Mr. Subhasish Das represented the Public Authority.
4. The Applicant was not present during the hearing.
Decision
5. The Respondent submitted that all the information was provided to the Appellant on 9.4.09. He produced a receipt signed by the Appellant stating that she has received the information.
6. The appeal is disposed off.
