Section 25(5) in The Maharashtra State Legal Aid and Advice Board Rules, 1981

(5)An application for legal aid and advice, if not in the prescribed Form, shall contain all the necessary details and information to enable the committee to decide the nature of aid and advice to be given to the applicant.
