Section 5(3) in The Orissa Clinical Establishments (Control and Regulation) Rules, 1994
(3)In the event of death of a patient in a clinical establishment the Director, Managing Partner or the Proprietor of the Establishment, as the case may be, shall furnish information to supervising authority in Form 3 forthwith.
