(3)he furnishes with the parcel a declaration stating the names of the consignor and consignee, the numbers and dates of licences held by them, the contents of the parcel in detail, the number and date of the permit covering the transmission and such other particulars as may be prescribed from time to time by the Excise Commissioner;
