Section 70(a) in The Orissa Municipal Employees' Pension Rules, 1989
(a)transmit to the applicant in Form No. IV a certified copy of certificate contained in Part II of Form No. Ill and one copy of Part I of Form No. V which is to be filled in by the applicant before his medical examination and to be handed over to the medical authority; and
