Every superintendent of Vaccination shall submit to the Director of Public Health, Bihar a weekly statement showing the incidence of small-pox in the following form and send a copy of the same to the Assistant Director of Public Health of the area concerned:-Statement showing the incidence of Small-Pox during the week ending the.............| Names of thanas affected. | Total number of villages affected up-to-date. | Total number of villages recording small-poxduring the week. | Total number of new villages affected during theweek. | Attacks. | Deaths. | Staff employed | Number of primary vaccination. | Number of secondary vaccination. | Remarks. |
