(2)An application for grant of a provisional registration certificate shall be accompanied by-(i)copies of passport size photograph of the proprietor or such of adult partners of the firm, or of such adult co-partners of the Hindu undivided family, as the case may be, duly attested by a lawyer, a tax practitioner or a Gazetted Officer; and(ii)An affidavit and a treasury receipted challan for rupees five hundred.
