(1)In the heading of the history-ticket of every prisoner, the Medical Officer shall enter or cause to be entered under his supervision -(a)the prisoner's weight on admission.(b)his state of health.(c)the class of labour for which he is fit, if sentenced to labour, and(d)whether he has been protected by vaccination, inoculation or small-pox etc.
