(1)A person intending to obtain or renew a license or approval under these rules shall submit an application, in writing, to the Chief Controller or Controller,-(a)in Form 'B' along with following document for a license prescribed in sub-rule (1) of rule 51; and(b)in Form 'C' along with following document for a license prescribed in sub-rule (2) of rule 51.
