A public vaccinator shall (i) maintain a register of children requiring vaccination in the following form (Schedule XXXI-No. 11H)in which the names of all newly-born and other unprotected children shall be entered, village by village or mahalla by mahalla; (ii) keep the said register up-to-date by obtaining relevant extracts from the birth registers, maintained at the police-stations; and (iii) note in the remarks column of the register mentioned in clause (i) all deaths of unvaccinated children against their names.
