(7)If an identity card is lost, mutilated or the pages contained therein are exhausted, the beneficiary concerned may apply in Form XXXIII to the officer specified in sub-rule (1) for issue of a Duplicate Continuation Identity Card. The application for a Duplicate Identity Card shall be accompanied by a fee of Rupees Twenty Five only payable to the Board in cash or an account payee Demand Draft.
