(2)Where a document is produced by a witness at the instance of a party the number of the witness shall be endorsed thereon e.g. Ex. 1/P.W. 1 it it is produced by the plaintiffs first witness, and Ex. A. 1/D.W. 1 if it is produced by the defendant's first witness.
