Section 27(2) in M.P. Rights of Persons with Disabilities Rules, 2017
(2)The following documents shall be attached with the application :-(a)Proof of residence;(b)Two recent passport size photographs; and(c)Aadhaar Number or Aadhaar Enrollment, if any.
