Section 812(a) in Police Regulations, Bengal , 1943
(a)An officer of and above the rank of Deputy Superintendent who wants leave or extension of leave on medical certificate shall forward to the Inspector-General through his immediate superior an application for permission to appear before the Medical Board, supported by a certificate and a statement of his case in triplicate in the prescribed form to be furnished by the medical officer attending him.
