A certificate to show that he has been appointed to work as an apprentice to a licensed public vaccinator shall be granted to every apprentice vaccinator in the form (Schedule XXXI-No. 9H) under the signature of the Superintendent of Vaccination. The counterfoil shall be kept in the office of the Superintendent of Vaccination :-
