(d)Application with authenticated documents along with First Information Report and report of the Medical Board of the District for availing financial assistance to the family of pouring member who died in fatal accident under these rules may be submitted to the Chairman of the Committee in Form No.III, which shall be accompanied by a report by concerned Deputy Registrar (Dairying) in Form No.IV;
