12. Grant of Duplicate Licence, Certificate and Permit.
(1)Application for grant of duplicate Licence, Certificate or Permit shall be submitted in Form-C.(2)A duplicate Licence, Certificate or Permit, as the case may be, granted under these rules shall be issued on payment of the fee as may be specified by the State Government.
