Section 4(1) in The Maharashtra Homoeopathic and Biochemic Practitioners' (Registration of Enlistment) Rules, 1961
(1)Any practitioner desiring to have his name entered in the Register under sub-sections (3), (7) or (8) of section 20, shall apply to the Registrar in Forms 'A', 'B' or 'C', respectively.
