Section 69(b) in U.P. Zila Panchayats Service Rules, 1970
(b)The verification of the service-books shall be taken up in the month of January each year by the head of the department who. after satisfying himself that the services of the servant concerned are correctly recorded in his service-book, shall record and sign a certificate in the memorandum of verification in Form No. 15 of the Financial Handbook, Volume II, Parts II to IV, which shall be made part of the service-book;
