(2)The application for claiming the amount specified in sub-clause (1) 1 shall be in Form No. VII and shall be accompanied by the death certificate of the deceased registered member and the original Identity Card issued to the deceased registered member. The original Identity Card shall be returned to such other registered member(s) after making due entries regarding the death of the deceased registered member and due authentication made therein by the Special Tahsildar (Social Security Scheme) for further use of the existing members.
