(5)(i)The electrical supervisor or the substitute electrical supervisor appointed under sub-rule (4) to replace him shall be personally responsible for the maintenance at the mine or oil-field, of a log-book made up of the daily log-sheets prepared in the form set out in Annexure XII.(ii)The results of all tests carried out in accordance with the provisions of sub-rule (3) shall be recorded in the log-sheets prepared in the form set out in Annexure XII.]
