(a)Any person who wishes to become a voluntary boarder in Mental Hospital must obtain from the Superintendent a form (Appendix 1), which he will fill up and sign and have it countersigned by two of the official visitors. He should then submit the form to the Superintendent who will admit him for treatment, if suitable accommodation is available.
