Section 5(1) in Sikkim Nationalised Transport (Prevention of Ticketless Travel and Miscellaneous Provisions) Rules 1983
(1)The forwarding note shall be prepared in quadruplicate in Form A and shall be signed by the consignor and the member of the staff of the Sikkim Nationalised Transport.
