Section 4(1) in The U.P. (Regulation of Building Operations) Regulations, 1960
(1)An application referred to in Section 7 to the Act shall be made in Form 'A' of Schedule I to these Regulations to be obtained from the Prescribed Authority on payment of such charge not exceeding Rs. 1 per form, as the Controlling Authority may fix.
