4. Application Form of scheme.
(1)Any person intending to establish a medical college shall submit the application online in Form-1 as amended from time to time along with non-refundable application fee.(2)Any medical college intending to open a new or higher course of study or training shall submit the application online in Form-2 as amended from time to time along with non-refundable application fee.(3)Any medical college intending to increase its admission capacity in any course of study or training shall submit the application online in Form-3 as amended from time to time along with non-refundable application fee.
