Section 36(1) in The U.P. Pravidhik Shiksha (Admission Conduct of Examinations and Conferment of Certificates and Diplomas) Regulations, 1992
(1)Application forms for admission to the Board's Examination for a regular candidate shall be obtainable from the Principal of the institution who shall act as forwarding officer for him. Candidates intending to appear as a private candidate shall obtain the application form from the Principal of the Institution last attended by him who shall be the Forwarding Officer for him. The application form duly filled in and completed in every respect along with the prescribed fee must be submitted to the Forwarding Officer by the date mentioned in Schedule-II. An application form incomplete in any respect is liable to be rejected by the Board.
