(1)The forms of declarations referred to in sub-section (2), sub-section (3), sub-clause (i) of clause (a) of sub-section (4), sub-section (6) and sub-sections (7) of section 8 shall be in Forms C, D, D-l, D-2, D-3 and D-4, respectively.(2)The forms of notices referred to in the second proviso to sub-section (3) and sub-sections (5) and (7) of section 8 shall be in Forms D-5, D-6, D-7 and D-8, respectively.
