(3)A record shall be kept at every rescue station of all persons undergoing practices and receiving instructions. Such record shall contain-(i)the date and the character of each practice;(ii)the conditions of each person after the practice, and if anything abnormal was observed in his condition, whether it was due to a defect of the apparatus or to that person; and(iii)any other relevant information.
