Section 39(c) in The U.P. Narcotic Drugs Rules, 1986
(c)The parcel shall be accompanied by a declaration stating the names of the consignee and the consignor, the contents of the parcel in detail, the date of the pass covering the transmission and such other particulars as may from time to time be required by the Excise Commissioner.
