Section 10(1) in Gujarat Professional Post-Graduate Medical Educational Courses (Regulation of Admission) Rules, 2018
(1)The candidate shall submit the self-attested copies of the following documents with the Application form, namely:-(i)All year MBBS/BDS Mark sheet with all Attempt Certificate;(ii)NEET-PG Mark-sheet of the current year;(iii)Eight Month Internship Completion Certificate;(iv)School Leaving Certificate or Transfer Certificate;(v)For In service candidate, study leave/resignation or NOC of Government of Gujarat;(vi)Certificate of registration with Gujarat Medical / Dental Council or Medical Council of India / Dental Council of India;(vii)Caste certificate for a candidate belonging to Scheduled Caste (SC), Scheduled Tribes (ST) and Socially and Educationally Backward Classes (SEBC), issued by the authority empowered by the Government in this behalf (for SC, ST AND SEBC candidates);(viii)Non-Creamy Layer (NCL) certificate of the family, as per the prevailing policy of the Government, issued by the authority empowered by the Government in this behalf (for SEBC candidates);(ix)Certificate of Physical Disability, issued and duly signed by the medical board constituted or medical officer authorized for the purpose; (for physically disabled candidates);(x)Certificate regarding Medical Fitness; and(xi)The documentary evidence of NRI status for candidate who desires to take admission on NRI seats.(xii)The necessary proof regarding his minority status and his willingness in writing to take admission in the concerned minority college / institution for candidate belonging to the minority community.
