(1)Every person entitled under section 34 to be registered under the Act and desiring to have himself so registered shall apply to the Registrar in Form D properly filled in and signed. Every such application shall be accompanied by the fee prescribed in that behalf in rule 73.(2)Where the application under sub-rule (1) is for registration under clause (ii) of section 34, the applicant shall state in the application whether he has passed the examination referred to in that clause as one of his qualifications for registration.
