Section 26 in Punjab Boiler Operation Engineers' Rules, 2001
26. Form of application.
(1)Every application for examination shall be in Form 'A' appended to these rules.(2)The applicant shall fill and sign the Form in the presence of a Gazetted Officer who shall attest the applicant's signature. The application so signed shall be forwarded to the Member- Secretary at least thirty days before the date fixed for the examination and shall be accompanied by :-(a)a copy of the degree or diploma, as the case may be, attested by the Principal of the concerned Institution or a Gazetted Officer alongwith the application for registration;(b)original and one copy each of all the testimonials in respect of practical experience of the candidate;(c)testimonials of good character from the employer;(d)an attested copy of the certificate of age;(e)demand draft in support of payment of the fee specified in rule 24 for the examination;(f)any certificate granted to the applicant under these rules or a certificate granted by a Board of Examiners for another State Government or a Training Institute, as the case may be, for the purposes of endorsement referred to in rule 23; and(g)two copies of recent bust photograph (size 50 mm x 63 mm) one of which shall bear the signature duly attested by a Gazetted Officer.
