Section 6 in The Orissa Pharmacists (Special Examination) Rules, 1962

6.

(1)The application for admission to the examination shall be submitted to the Secretary in Form I within such time before such examination as may be notified by the Secretary and shall be accompanied by-
(a)The Original Compounder's pass certificate;
(b)A declaration by the candidate that he has attained the age of 18 years before commencing the work as a compounder;
(c)An examination fee of Rs. 20;
(d)Two copies of passport size photos of the candidate, one of which shall be attested by M.L.A. of the constituency by any of the following authorities under official seals ;
(1)the Chairman of the Municipality concerned if the applicant resides in an urban area;
(2)the Chairman of Zilla Parishad or Panchayat Samiti concerned if the applicant resides in rural area;
(3)a Gazetted Officer of the State Government;
(4)a Member of the Orissa Medical Council or Pharmacy Council.
(2)The attested copy of the photos shall be retained by the Secretary of the Committee for identification of the candidate in the examination hall. The Secretary shall issue admit card to each candidate, qualified to sit for the examination in Form II, on which the other copy of the photo shall be fixed.
