Section 6(1) in The Orissa Pharmacists (Special Examination) Rules, 1962
(1)The application for admission to the examination shall be submitted to the Secretary in Form I within such time before such examination as may be notified by the Secretary and shall be accompanied by-(a)The Original Compounder's pass certificate;(b)A declaration by the candidate that he has attained the age of 18 years before commencing the work as a compounder;(c)An examination fee of Rs. 20;(d)Two copies of passport size photos of the candidate, one of which shall be attested by M.L.A. of the constituency by any of the following authorities under official seals ;
