Section 29 in The Orissa Boiler Operation Engineers' Rules, 2000
29. Form of application.
- Every application for examination shall be in Form A. The applicant shall fill in such parts of the Form as are to be filled in by a candidate and shall sign the form in the presence of a Gazetted Officer or his employer who shall attest his signature. The application so filled in shall be forwarded to the Member-Secretary at least two months before the date fixed for the examination and shall be accompanied by :-(a)a copy of the degree or diploma, as the case may be, attested by the Principal of the Institution or by a Gazetted Officer and the original and one copy each of all testimonials in respect of practical experience of the candidate;(b)testimonials of good character from his employer, with certificate of age;(c)a treasury challan in support of payment of the fee specified in these rules for the examination at which the applicant wishes to appear;(d)any certificate granted to the applicant under these rules or a certificate granted by Board of Examiners from other State Government or a training institute for the purpose of endorsement referred to in rule 25; and(e)two copies of recent photograph (size 50 mm. x 63 mm.) one of which shall bear his signature duly attested by a Gazetted Officer.
